Fees may be paid at the school web store or in the high school activities office.
Fee waiver information provided in the registration.

SPORTS PHYSICALS
Please turn in only the FIRST page of the form to H.S. activities office.
(Entire physical form is to be kept on file with your healthcare provider)
Physicals due every 3 years.
Please contact your healthcare provider if you need a copy.

To pay by check or cash, use a computer to register, or drop off a sports physical,
please stop in the Activities Office
between 7:45 am - 3:45 pm Monday-Friday
